SUNY Delhi Men's Basketball: Clinic Registration Form

Register for SUNY Delhi Men’s Basketball Clinic being held on July 20-23rd, 2026.

Ages: Open to boys and girls entering 4th-10th grade in fall 2026

Time: Check-in starts at 8:30 am, clinic runs from 9:00am – 1:00pm.

Place: The clinic will take place at Floyd L. Maine’s Arena, inside the Clark Field House on SUNY Delhi’s campus.

Cost: $150 per participant (Payment accepted on the day of the event by cash or check, made payable to “The College Foundation at Delhi”).

Registration Deadline: Registration is open until July 13th, 2026. If you are looking to apply after the deadline, please reach out to Coach Annan directly to see if there is any availability left.

Lunch: Each day free pizza will be provided at 12:00 pm. Additional snacks and beverages will be available to purchase.

1.) I am the parent/guardian of the above-named athlete, who wishes to participate in the clinic sponsored by SUNY Delhi. 2.) I herby give my permission to SUNY Delhi to allow my child to participate and that I agree to assume risk for all injuries suffered by my child as a result of participation in this clinic. 3.) I agree to indemnify and hold harmless SUNY Delhi, its employees, agents, and volunteers for any and all causes of action brought against SUNY Delhi which are commenced by third persons alleging injuries, property damage, or death arising from the acts of my child, whether negligent, malicious or intentional. 4.) In the case of an injury, I authorize the staff of SUNY Delhi to provide initial first aid. 5.) SUNY Delhi shall not be financially or otherwise liable for any care beyond providing initial first aid, regardless of whether additional services are covered by my insurance. 6.)
